How to locate the form?

Hi support team,

I have a form but I don't know where it is located is there a way I can see where it is link to? Cause I know we only use 3 forms in our website

when I click view form this one appears

but I don't remember this form appreciate if you can help me on this

You can add a "Get Form Page URL" widget to your form:

http://widgets.jotform.com/widget/get_form_page_url

Each time the form is submitted, the widget will report the exact page the form is submitted from.  The next time someone submits the form from your webpage, the page will be reported back.
