Just testing - but when I enter a quantity and press submit - in the notification email I get the template header - with the fake company - not my form header that I designed. none of my logos or anything shows up.

You can edit your email notification template. It was probably carried over from the original form if you are using a cloned version of someone else form.

You can also delete the existing notification and create a new one, this is pretty much easier.
