A form will be in restore mode whenever the account becomes Dormant. It appears that your account has not been accessed in a long time, which is why your account was flagged as Dormant. I have now reactivated your account so you should be able to use your forms successfully. Cheers.

if you didn't login for a long time, your account is on dormant status. It has to be reactived.
I also suggest you to test your forms after reactivation.
