I wanted to make a pdf of my form.. when i do my checkbox fields do not appear on the form

Checkbox fields are supported on fillable PDF.

Are you perhaps referring to the Checklist Widget? I can see lots of them in your form. Unfortunately, widgets are not supported on fillable PDF by nature. I don't think there's a way to overcome this other than replacing your checklist widget with a regular checkbox fields.
