Getting lots of multiple submissions on my forms, causing our submission limit to be reached for the month. Is there any way to know if this user error or if there is some sort of bug? This is happening with both "New Patient Online Form" and "Medical Release Form"

Since form submissions have already been deleted and encryption was set for most of them, we cannot check the contents to see the duplicates. However, according to your forms submission history, the submissions were indeed unique submissions and not replicating on our end.
If you would like to limit users to a single submissions for each form, it is possible to set up a unique submission check to prevent duplicates:
