Form has started duplicating words and letters within the answers on the document.

    I'm not quite sure how to explain what it's doing, but once you download the PDF file and the form appears on your screen, certain words and numbers are duplicated within their respective line.  It's adding hashtags in various text as well.  Please assist in resolving.  We depend greatly on these forms.
