Is it possible to add a box to take a photo?

I was wondering if there was a tool that I can add into a survey to take a photo on the spot.

basically I guess my real question is it possible for an option to appear that gives option to access camera to take a picture or access camera roll when tech is filling out survey on-site

I understand that you want to access the camera to take photos from the form. If so, you must be looking for Take Photo widget.

I tried that and at first it just put a black box on my survey now it says its no longer available on unsecure page whatever that means

I have tested the widget on one of my forms and seems to be working fine at my end. Could you make sure that you are accessing the form using https in the URL? Please embed the widget on your form and provide us with the form URL so that we can take a look and provide you with necessary assistance.
