Why has my form disabled

    I just open an account. I completed my first form and it has been disable after I published the form. Why is this happen. Is there any settings to enable the form.

    I see that your account is auto-suspended by our anti-phishing system which is strict to scan each JotForm to calculate the probability of phishing activities.

    I have manually checked your Form, and found you are using Password fields on your Form. We do not allow password fields or sign up forms that contains passwords and usernames. It violates our Terms of Use as this kinds of forms had been extensively abused by some of our users to conduct phishing or other illegal activities.

    I would suggest you to remove password fields from your Form, and I will activate your account again. Thanks.
