My Form has been disabled

    Hey,

    I was working on a new form and I went preview it to see how it was coming along and it said my form was disable.

    There were several terms in your "Claim Application" form that set off our phishing detector.  References to gaming platforms have an especially high impact.  I checked your form and it looks to be within our terms of use.  I whitelisted the form and activated your account.  It should no longer cause you any trouble and your account is active once again.
