It looks like there are some resources that are not loading, then the form looks like that.
Could you please provide us a screenshot of the web console?
These keyboard shortcuts will help you to get the console in Chrome:
Mac: Command + Option + J
Windows: Control + Shift + J
Also, could you please try loading the JotForm page on another computer?
This happens when there are some network problems or resources are being blocked for any reason, please let us know how it goes testing on another computer, and provide us the screenshot it would helpful.
