Good Morning I wonder if you have any form to appointments ? I'm gonna explain: Have an average clinical and precise that my clients mark the time for a form. When the customer reserve certain time becomes unavailable to other people . Thank you very much

You can use one of our widgets called Appointment Slots widget. In this widget, you can limit how many people choose an available slot because the widget reduces the number of selected slots each time the form is submitted. Here's a demo form.
Also, we have available form templates related to appointment that you might be interested to.

Unfortunately, we do not have a widget that can cover specific time slot and mark them as unavailable.
However, you can set specific items with time slots instead in the Appointment Slots widget. Here's an example form: https://form.jotform.com/62355178439969. Here's a screenshot of it:
So basically you are setting each item to have the day and the specific time that can be reserved or set an appointment to.

When a patient choose a the same time is inactive for other clients ?
Can I put next to the days of the week the dates ?
If any patient cancel the time how do I reactivate it ?
How to start a new calendar ? Example : I will change month.

I would like to suggest testing the widget so you will get an idea how it works.
Can I add more time?
Yes, the slots (Appointment times) are not automatically created. You will need to manually add all the slots you need.

Yes, this is correct.
Can I put next to the days of the week the dates?
We do not have such option.
However, you can edit the text of the slots to specify the dates.
If any patient cancel the time how do I reactivate it?
To do this you can increase the number of available items. For example, if it was 1 spot, once canceled you will need to increase it to 2.
How to start a new calendar? Example : I will change month.
You can configure a new widget field, or duplicate your current widget.
