the customers are saying that the form does not work as well we can not get!

I did check your form which you have embedded in your webpage and did not find any issue with that. I was able to submit it successfully and I was redirected to thank you message. Please check the screenshot below:

Yes I did thank you very much ,I'm very glad I finally was able to find a communication source with you guys ,I need your company help and we have no problem to pay for the good services your company has to offer .
I need to establish a feed back form with 5 star rating on the web site and also a form where we can email to clients to provide feed back to us regarding the service .
Alsoamong other forms that we may need to add on the site .
Please advise on how to proceed .
