Form fields and images not showing in the notification email

I tested my form and most of the values I filled out or the photos I uploaded don't occur in my submission mail.

I cloned your form and I found out that you email notification is empty. It is empty because your form only have the heading and the submit button.

You are probably referring to a different form. However, this is the only form I saw under your account.
