    I have lost my user name (PMX?) and password - can you help me?

    I bought a 2-year subscription

    PMX account is still active to PREMIUM under pmx*******.com and upon checking the email you provide, there's no user found.

    You can use Forgot Password button at www.jotform.com and on the bottom right of the page:
