The form that I received via email is not complete but when I double check in jotform and in the website the form is complete and well done.
Please help on how to solve this.

I checked your issue but the form at your website is a different form that screen shot above. Could you please recheck and give us some more detail. In addition to that, you can embed your form to your website by iframe. Below link can guide you.
