Hello,
I am a college professor, and I fear I have "autolocked" my account by creating a form that asked for students' usernames and passwords. The form was going to be used as part of a class project. When I tried to view the form I got he message "This form has been disable." It also seems that other forms in the account are also now disabled.
From what I have read online, the JotForm system monitors forms for words that suggest some type of "Phishing" activity. What do I need to do to have my account reset?

While reviewing your account , I did not notice any of your two forms violate our terms. I am assuming that you either removed the form or adjusted the form properly so it will not violate the terms.
Since there is no violation, I have reactivated your account.
