    The fillable PDF creator will automatically add the notification email in the PDF form. The recipient will be your primary email address, the one that is registered in your account. But I've checked your PDF form and it seems that it's not a fillable one. It's just a static file with no fillable fields, print-only file in general. 

    Do you still have the original file? Can you send it to us? Please send the file to support@jotform.com, if possible to share. Please don't forget to mention this thread in the email and let us know here after so we can check it right away.

    I have checked your PDF file which you sent on email. But it is not a fillable pdf file. It is just a static pdf file.

    So I've done this multiple times and when I upload my PDF to a fillable form its still not fillable.

    Hello,

    As mentioned by Welvin and irshad, If you are trying to upload a PDF form which does not have the capacity to be fillable, unfortunately that will not work.

    So to explain -- what the JotForm fillable PDF creator/processor does is either it creates a fillable PDF based on the webform created, or it captures the actual fillable fields from the original PDF that was uploaded. So if your form does not have fillable fields in it, it will not work.

    Hope this clarifies some issues.

    With these being said, I would recommend that you create your web form via the form builder and then download a PDF so you get to have your fillable PDF form.
