I set up a form, but when I go to preview how it works I am getting an error message. It says the form is disabled. But the settings say it is enabled.

Hi Lorraine, Your account was autosuspended by our phishing filter by accident. It's probably because of how this form's header was labeled - It says "Vendor Contact Information"
I manually checked ALL the forms you have on this account (including the ones on your TRASH folder) and confirmed that they are all clean and are not being used for Phishing. Please avoid using Contact Information as your label (or any word/term that pertains to self-identifying contact info, sensitive info, credit card details, account details, usernames, passwords and so on)
We're sorry for the troubles - I have reactivated your account so you should now be able to start working on your forms again :)

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
