I've changed the preferences to check for cookies and IP in order to prevent multiple submissions, but we're still seeing multiple submissions on almost every submit. Any ideas what could be causing this? We're seeing on both the forms on this web page below.

I checked the mail logs for one of the recipients, and there was no duplicate submissions:
