i am an interested user in this service. i own a pediatric dental practice where we have many forms we have patients/parents fill out. we would like to transition to an electronic platform and avoid use of paper.

Our paid plans start from $19.00/month for up to 1,000 submitted form entries per month. We also have hight volume plans, please take a look at our pricing page for additional information:

The service can be tested without upgrade since we offer a free plan (up to 100 form submissions per month) which is not limited in terms of features as of this time.
