Jot form Down

Is the form down, when I click on the http://www.jotform.us/form/22509214695153, we get a form is currently down. 

That form is currently manually disabled.  While editing your form, click on the "Preferences" button, then navigate to the "Advanced" tab:

Change the status from disabled to enabled to active the form once again.
