Form Broken?

    One of my forms (Farmers' Market Vendor Application) seems to be not functioning. See below. The second picture is the error received when trying to access the form via the link (https://form.jotform.com/20778080228152)

    When checking your form, I noticed that it was disabled.

    I have enabled the form and you should be able to view and submit forms now. Please let us know if you still require our assistance.
