Browser - form not available message

Tried to access "My forms" an the site looks completely hosed -- no formatting at all.  Did find this contact email.  This is an existing form with no changes on our part.  Please advise status of the site. 

Apologies for any inconvenience. We had an issue earlier which was addressed to our developers right away and is now fixed. Please check your account and forms again as everything should be working as it should.
