The form I created isn't saving my information correctly.

I have created a form using a template. It is exactly what I need. However when I go to save, the wording in the text field is still using the wording from the template (Erin McCoy) and not my own contract (Tabby Z). What am I doing wrong?

I am not fully understanding, can you please indicate what field or fields are not getting updated. I have reviewed the form and see that you have added the image logo and html text. Can you please indicate what wording is not correct?
