I made a copy of the form I used last year. I have enabled it, but when I fill out the form all it says is Please Wait. Since it has been since February since I've done this can you give me a reminder of what I need to do to go live?
Also, how do I get the logo smaller, I reduced the size on the template but it doesn't show.

I have tried submitting the form using the direct URL and the submission went fine. However, when I tried to submit the form embedded on your web page, I am able to see that the form hangs at Please wait.
It looks like that you are using WordPress for your web site and the form is being embedded using a WordPress plugin. Could you try embedding the form using default form embed code.
Also, please disable the form embed plugins to avoid any conflicts to display the form.
