Form not laying out correctly.

I am having trouble getting my form to layout the same way it does on your platform. Even when I put a template up with no edits, the layout gets messed up.

Please see here - this form - all I did was load the template and embed it on my site - I tried using both the embed code and the specific wix code and the quantity line indents.

The Quantity field is currently shrunk, and since it doesn't have any other fields next to it, the field automatically adjusts to the middle. To resolve this issue, simply click on the gear icon of the field and select Expand.
