Integrating forms onto squarespace

I am trying to embed my forms onto squarespace, but when I try to do it using the instructions provided it cuts off half of the form including the submit button. How can I fix this?

I tried my luck Googling around hoping to find your website using "theheragency" as my search keyword but failed to locate one. I found one http://theheragency.com/ but this appears to be a parked (and blank) GoDaddy domain.
