I lost all the submissions in one of my forms

I linked the forms to my website a month ago and it was working just fine but just now I checked on my website it said that the URL cant be found and when I opened my jotform, the form is still there but the submissions are all gone and it still cant be opened from my website. What happened? Can someone please help me in this:(((( thank you
