I can't link the form to my website

It was working just fine since I linked jotform to my website last month, but then just now when I opened it from my website it said the url cant be found. I tried to re-do it again as publish the form and copied the html to my website but it still didnt work. Help please?

I checked your webpage however, I can't see any JotForms form embedded on it. I can see that you are using Tumblr, please check this guide: Adding-a-form-to-Tumblr

If the normal embed code didn't work, please try re-embedding the form using Iframe method.
