Why should I take your site form ?

If I'm paid user and I made an order form then form url (ex. www.jotform/contact_us) shows of your site. But I want it should be on my url link. (ex. www.mywebsite.com/contact_us). 

Is there any possibility that I can download full form and upload on my server and so it can show by my url.

Otherwise my customer/client when click on your form they will surprise to see different url and they can think that they come up on different site with different url.

You can embed the form on your website page so that you can use your own website URL to show.
