When I do a test on my form, the template I am using still sends over the old photo as the header instead of my photo.

    How can I get rid of the templates logo/photo as the header and make sure mine shows up?

    I removed theirs and added mine. On the form itself, it shows mine, Melissa Badertscher Photography, but when the form is submitted, it shows the old one that came with the template of Beyond the Box Photography, which is not mine.

    The logo, "Beyond the Box Photography", is  inside the content of your email notification. You need to manually update it with your own logo using the toolbar(s) available when you compose the email notification content.

    You need to also do this with the email autoresponder in your form.
