My form doesn't show

My forms are showing as "Form not Found, this form is disabled" and doesn't appear on our website !! can you please tell me why this is please ??

I checked your account and it seems that your account was automatically suspended because one of your forms contains a Password field which is also the reason why you cannot access your form.

Collecting sensitive information such as Password field is against our terms of use. I have deleted the password field on your form so I can activate your account. Please avoid adding a Password field in your form to avoid your account to be suspended again.
