How to Embed a Form into Facebook Page

I am trying to integrate an application form into our Facebook page but the instructions you have are out of date.

Could you send a new instruction outline of how to do it please?

I just tried the following instruction and it is up to date. You just have to use the Facebook option under Platform on the Publish Wizard.
