Getting Error: 'Credit card number is required' when processing a form with credit card

Getting this error on all forms with a credit card processor.  We are obviously entering a correct CC #. 

Was working fine as of yesterday. We tried clearing website cache, and re-setting up the form, but that did not fix.

Please provide support as we are no longer able to process payments on our website.

Please help us resolve this asap!
