    cannot get my document to upload

    I click upload file and upload it as a pdf.  It shows up but does not show up on the form

    It seems that you haven't made any submissions yet. I also noticed that the form that is on your website doesn't have a submit button. You will need one for all the details to be sent.

    If this isn't the case, please provide us the Form ID for us to check.

    I NEED TO COMPLETE MY FORM AND ADD A WAIVER TO IT.  I HAVE IT AS A FILE ON MY COMPUTER.  HOW DO I GET IT ON THE FORM.

     

    PS  I HAVE A SEND BUTTON INSTEAD OF A SUBMIT BUTTON ON THE FORM

    If you are planning to have a waiver on the form, you might want to have a look at this workaround.

    The upload button is for uploading files to be sent, it isn't for embedding purposes.

    Yes, I can now see that you've added a submit/sent button.
