Form not available on mobile facebook?

When I go to my business page on FB from my cell phone I cannot find the contact form that I have added on using JotForm... when I look from my computer, it is definitely there. 

Why is it not on my mobile page? How do I fix that?

I have tried the static method.. I have ensured that my form is "mobile responsive." I can't seem to get it to work on my cell phone at all... 

Unfortunately, the "Form tab" method does not work because the tabs are not setup to be mobile friendly by FB yet, here is a discussion about it: https://developers.facebook.com/bugs/435173473172584/ 

You could instead add your form through the Custom Iframe App method: https://www.jotform.com/help/82-Adding-a-form-to-Facebook-Custom-iFrame-App-, since Static Iframe Tab App provides direct URL to your page tab that is viewable on mobile devices.
