The system does not update modified email templates. The email template on your form is most likely outdated. In order to fix this issue, please re-create the Email Notification.
1) Delete current Email Notification.
2) Add a new Email Notification.
This will create a brand new Email Notification with your current form fields and enabled auto-update.
