My form is not displaying correctly in Chrome or Safari

    I'm not sure what is happening. My form displays correctly in Firefox, but not in Chrome or Safari. Here's the page it's embedded on - http://enlightenedprofits.com/jotform/

    Hello @gaintheedge - I checked your website on both Chrome and Safari (even tried with Firefox just to be sure) and it loaded just fine. I also tried it with the stand alone version https://jotform.com/53335580827157 and that worked too without issues.

    Chrome 52.0.2743.116m (64-bit) on Windows 10

    Are you still having this same issue until now? If so, we'd appreciate it if you could share us a screenshot of how your form looks like from your end. Please don't attach it from your email, go to your thread here https://www.jotform.com/answers/901907 and attach it from there (here's how).

    And if there are any special steps we need to take to replicate the problem, any additional info will be helpful. Hoping to hear from you soon.
