We are not currently receiving form submissions for our Pet Form, and haven't received any since August 3rd (we usually receive a dozen or more per day).
The Pet Form displays for any Pharmacy product once you click Add to Cart, here's an example product:

The form is displaying when clicking Add to Cart, but JotForm is not receiving the submissions.
I checked your page and indeed, there is an issue with submitting the pet information form. It looks like something in your page is blocking the form from being able to complete submission:
Try embedding your form once again. By default forms now use https and adding the updated code to your page should allow for the form to submit without issue.
