Why is my form over quota?

    I recieved 3 submissions on a form and now am getting a message that the form is over quota - those are the only submissions this account has recieved this month  - why is this happening? Please fix it.

    I checked your account and you are far from reaching the account limits, you can check your account limits by clicking the human icon shape at the top right corner of the page or by simply navigating to your Account->Usage page.

    I also went ahead and recalculated your monthly usage just to make sure.

    I was also able to submit your form successfully. Here's a screencast of it:
