Why is my account auto-suspended?

    Hi, I'm trying to log in my account but found this error. Please help me in this regard.

    Your account has been suspended because of this form http://www.jotform.me/form/62215199356460 

    There you're asking for Credit Card information something that is not allowed at JotForm and it is considered as phishing.

    I had to remove the guilty fields from your form and un-suspended your account, please check and let us know if you have any question.
