Please advise if you have a form for the following.

I am creating a website whereby clients can request for transport.

So basically

1 The transporter registers the details.

2 A client comes onto my website and requests a quote, and there details automatically need to be forwarded to the service provider, or to a few service providers at the same time.

I will be selling leads to the service providers.

JotForm is an online form builder that let's you create your custom forms. It would be best to checkout the features of our product so that you can see if it will meet your needs. 

Now I am not entirely sure what is the process flow of your form, but we do have a lot of features if you are just looking for a way to forward data to another email address.
