How to create a JotForm form based on an existing Word documentAsked by Stephanie Robins on August 10, 2016 at 09:45 AM
I was recommended to inquire more about your site for my therapy practice. I have new client intake forms that all my clients are required to complete. Currently I have my forms in a word doc/google word doc. Is there anyone that can help me change the format to your program without me having to start from scratch? I can send what I am currently doing if that would be helpful. Thank you.
