How to create a JotForm form based on an existing Word document

    I was recommended to inquire more about your site for my therapy practice. I have new client intake forms that all my clients are required to complete. Currently I have my forms in a word doc/google word doc. Is there anyone that can help me change the format to your program without me having to start from scratch? I can send what I am currently doing if that would be helpful. Thank you.

    We do not offer services where we would build the forms for you, but forms are really easy to create with our easy-to-use Form Builder, and we are here to help out if you need help with any particular requirement.
