Form Disable

    Why my From is disable , how it will be enable .

    Our form Phishing detector have disabled your form https://www.jotform.com/52593018949466  due to some elements on your form it percieve as a method of phishing confidential or sensitive data.

    We need to remove those field on the form before can request for the re-activation of the form and account.

    Let us know if you agree to this proposition.

    can i create this field again..

    Hello jskus,

    I did check your form and I believe you should not have any issue in asking for PAN card, Aadhar numbers. They are used as ID proof in India and can be asked in online forms. It seems you have few form fields without any label which is causing our any-phishing filter to trigger. I have deleted fields with no label and activated your account.
