Is it possible to add a ''take a video'' in the form?

We need to be able to take a video. It one of the question in the form.

If you would like to upload the video file to the form that would be possible. All that you would need to add to your form is the file upload field: How to create upload forms

Unfortunately there is no widget at this time that would allow you to record the video directly, just to take a photo: Take Photo
