Is there a way to have this text show in our PDF format of a submission

    We added a body of text, a disclaimer that our services are not meant to replace a licensed healthcare professional. Is there a way to have this text show in our submission, so we have that on record?

    Also, I am having trouble with creating required fields in the form. I am not doing something right. I click on the "required" icon, but nothing seems to be happening.

    By default text fields are not shown in the PDF format of a submission. If you want that text fields from your form are shown inside of you PDF report you will need to check "Show Headers and Texts" option in the form submissions page. Inside of this guide you can find how to do it:
