Hi I am trying to figure out if this was a jotform glitch or an error from my Client, who also access the jotform account for reports.
They recently realized that a good chunk of questions were missing from their survey reports.
When I went to look at the actual form on jotform, I realized that at least 10 fields at the end of the form, including the submit button, were missing.
Is it possible at all that this error occured on jotforms end? I am of course leaning towards the client touching something and making a mistake, especially since it seems odd that only a certain chunk of the fields dissapeared, and all at the end of the form.

Do you mean they are missing from the form or from the report? If they are missing from the report you might just be not included the fields where you check the fields you would like to see on the reports.
When I look at your form I see a submit button.
Are you seeing it missing on the web page? If that's the case, can you post the URL of the page?

I mean that they were missing from the form, separate from the reports.
However now I have already replaced the missing form fields for the client, so now everything is working correctly, but a few days ago was when I went to the modify the form on jotform, the fields were most definitely missing.
It is highly likely the client did this by accident and didn't know what they were doing, but I wanted to see if it possible that it was a Jotform glitch before I told them that they were probably the source of the problem.
I searched the forum using a few different keywords, but this was the only occurrence of my problem I could find. Like this person, my fields just disappeared from the form builder. This is now the 2nd time this has happened, so if there is something causing it I would like to know so I don't do it again in the future.
I've been putting together a detailed employment application and most recently, the 20 or so fields at the end of the form were gone. Poof, just vanished. If one or two fields were missing I could see that being my error if I accidentally hit the delete button, but so many consecutive fields is no little mistake on my part. Something is going wrong. I'm the only one with access to my account. Has anyone else had this problem? Any solutions? Thanks for your time. ~Amanda
