How to prepopulate the Full Name field and Phone field using URL parameter

I need help in populating a Name fields which are divided to First Name and Last Name. Also doesn't work on Phone numbers.

I can't fill it using the Update / Calculate Field in Conditions. It's only possible when it's a plain text box. Is there any other way?

As you can see its a Full Name field and a Phone number field.
