I did finally remove the check in the box but the default seems to be the box automatically checked and at times even when I insert a box and “uncheck it” it returns with the check. It seemed curious to be the “check” would be the default. It seemed to be taking a fair amount of time to keep changing it.
Having said that I am working with your site (I was using Adobe Acrobat DC) and probably will ask my group for permission for a paid plan.

Your are welcome. Just as a suggestion, you might try clearing your forms cache: https://www.jotform.com/help/229-How-to-Clear-Your-Form-Cache. Hope that avoids the return of the default selection.
