    How do I make a check the box option that requires user to answer one or the other but not both (I consent to X or I do not consent to x)

    If your aim is to only allow the user to choose only one of predefined set of options, you should use a radio button instead of a checkbox

    A checkbox is a field that permits the user to make multiple choices while a radio button only allows one. Let us know if you'll need more help.
